COMMITMENT DAY IS HERE!

1. Print out and complete the SCSN Medical Release form: Click for SCSN Medical Release Form
2. Complete the SCSN Player Commitment Form (https://scsnvolleyball.sportngin.com/register/form/002922184)
3. Register for the 25-26 USAV / SCSN Membership & Complete Club Assignment from the site link below:
a) SCSN Clubs - USA Volleyball (https://usavolleyball.org/scsn/scsn-clubs/) Scroll down and select Angels Elite Volleyball Club.
4. Print out and complete the Club's Financial Agreement for the 2025-26 season and turn in the completed form to Natasha Hernandez. (Click for 2025-26 Financial Agreement)
5. Print out, read and review the Parent & Player contracts and turn in the completed forms by email or to Natasha Hernandez.

If you need assistance or prefer to drop off the paperwork in person I will be at Amistad High School gym on Friday, August 8th from 7-8:15pm.

(The above link will provide a list of all the clubs with their public club assignment link attached on the SCSN website. When the member clicks on the club link, they will purchase their player membership, and the club assignment will automatically be attached to that player.)

Parents & players are required to complete, turn in all paperwork, and make deposit payment
NO LATER THAN FRIDAY, AUGUST 1ST, 2025 to secure their position.
